1. Identification

A form of ID that shows name and date of birth. Examples include:

Driver’s license or photo ID card
Work ID
Birth certificate
School registration or ID
Any official document showing the date of birth

2. Proof of Residence

Documentation that shows your current address, such as:

Utility bill
Rent receipt or lease agreement
Bank or financial statement
Insurance document

If you are experiencing homelessness or do not have proof of residence, please come in with the information you do have. We will do our best to assist you.

3. Proof of Income

Examples of acceptable documents include:

Recent pay stub
W-2 or 1099 form
Employer income verification letter
Social Security benefit statement
TANF (Temporary Assistance for Needy Families)
SNAP (Food Stamps) documentation
Child support documentation

To serve as many families as possible, assistance is typically available at the following intervals:

  • Food: Every 30 days
  • Clothing: Every 3 months
  • Furniture: Large items once per year
  • Utilities: Up to twice per year
    • Once through Manna Storehouse
    • Once through Salvation Army–assisted funds
  • Prescriptions: Limited assistance available on a case-by-case basis (typically for insured individuals)
